Pomalidomide is a thalidomide derivative, a potent inhibitor of TNF-α production. It is an antiinflammatory and antitumor agent used in the treatment of multiple myeloma.

Pomalidomide In vitro studies:

Pomalidomide inhibits the release of TNF-alpha stimulated by lipopolysaccharide (LPS) and has an IC50 of 13 nM and 25 nM in human PBMC and human whole blood, respectively. Pomalidomide inhibits IL-2-stimulated T regulatory cells with IC50 of ~1 μM. 6.4 nM-10 μM Pomalidomide treats human peripheral blood T cells and increases IL-2 production, acting on the CD4+ subset more efficiently than on the CD8+ subset. Pomalidomide significantly promotes IL-2, IL-5, and IL-10 than CC-5013, and slightly promotes IFN-γ over CC-5013. Pomalidomide acts on Jurkat cells and enhances AP-1 and Raji cell-induced AP-1 transcriptional activity, which is the highest 4-fold increase at 1 μM. This effect is dose-dependent. Raji cells treated with different concentrations of Pomalidomide (2.5-40 μg/mL) for 48 hours resulted in a significant decrease in cell proliferation and DNA synthesis, which was reduced by ~40% compared to the control group.

Pomalidomide In vivo studies:

Pomalidomide acts on SCID mice and enhances the anticancer effect of Rituximab on B-cell lymphoma. The combination of pomalidomide and Rituximab resulted in an average life span of 74 days, and the average life span of mice treated with CC5013/Rituximab was 58 days. Depletion of NK cells results in the complete abolition of the synergistic effects of Pomolidomide and Rituximab, suggesting that NK cell proliferation is a mechanism by which Pomalidomide enhances Rituximab's anticancer activity.
